Kessler
Adults and Older Persons who receive mental health services may be asked to complete a questionnaire called the Kessler 10+ or Kessler L3D. A copy of this questionnaire is below.

Strengths and Difficulties Questionnaire
Youth aged 11 - 17 years, as well as Parents and Teachers of Children aged 4- 17 years, may be asked to complete a questionnaire called the Strengths and Difficulties Questionnaire (SDQ). There are different questionnaires that need to be completed at different times during a child's treatment at a Child and Adolescent Mental Health Service. Below are copies of the various SDQ's that might be completed by either Parents, Teachers or Youth.
