It is recognised that services may need to alter the forms to meet their own Medical Record Department requirements. Please Note: When printing the forms please ensure your printer is set up to print as A4 (not A4 letter/legal).

The K 10+ is the consumer self report form for Community Mental Health Services and is a mandatory collection requirement.
